Committee on Leaves and Privileges (CLP)
Use this form to request a second major. All requests will require permission of both the first and proposed second major chairs and advisors.

In order to declare a second major, students are expected to:
- Have a good academic record
- Have successfully completed two courses in each of the majors
- Have completed a plan for both majors via Degree Works or other planner as determined by the department/program. This plan must be approved.
